 Introducing Modality Partnership  Mid-Sussex


We are pleased to share that we will be joining with other local GP practices to form the Mid-Sussex division of a national GP group called the Modality Partnership.


This will take effect from 1 April 2019. 


The local practices that have decided to come together includes Crawley Down Health Centre, Judges Close Surgery, Park View Health Partnership and Ship Street Surgery.


Modality Partnership share our vision and commitment to NHS values and to provide high quality services for our patients.

If you find you can’t make an appointment please let us know ASAP. It can then be used by another patient. There maybe a time when that other patient is YOU.

How To See Your Doctor

child_sore_throatThere may be one Doctor whom you look upon as your personal Doctor. However, you may consult any of the Doctors at the Surgery. This may be necessary if your own Doctor is unavailable (on holiday or study leave).

For ROUTINE appointments or queries about results, please avoid telephoning between 8.30am and 10.30am if at all possible. It is usually much easier to telephone the Surgery in the afternoon.

All patients requiring medically urgent care will be offered a same day appointment.  Appointments for routine or non-urgent consultations will be booked for you, by a Receptionist, for a mutually agreed date and time.

Keep your appointment

Please do everything you can to keep appointments. Tell us quickly if you cannot keep an appointment either by phoning the surgery or using the online services appointment cancellation option.

There is an automated check-in system to avoid queuing at the desk.  If you arrive later than your appointment time this may cause delays and inconvenience to other patients.  If you are more than 10 minutes late, it will probably mean that you will have to rebook.  If we are running late please be patient – on another occasion, it may be you who needs the extra time.
