Are you a carer?
If you are, let us know - we maybe able to help. We have a Carers’ Support Worker attached to the Surgery and if you would like her to contact you, please ask at Reception or speak to a practice nurse or your GP.
If you are a carer, you can ask to be included on the Carers’ Register, which will entitle you to, amongst other things, a flu injection and an annual health check. Please complete the form below and return it to the surgery.
